At the end of September, a recruiter posted an opening for SDRs at "one of the biggest names in healthtech right now": a company with a $7B valuation, backed by General Catalyst and Sequoia, and working with more than 500 healthcare organizations. The offer was $90K-$110K OTE for SDRs with just 3-6 months of experience, plus an accelerated path to an AE role in 6-9 months.

That posting says a lot about the market for healthcare sales development talent. Well-funded companies are paying up for reps, and they're promising those reps a fast exit from the SDR seat. If you're a healthcare B2B company deciding whether to build an SDR team or bring in a managed one, the salary line is the easiest number to see and the least complete.

This guide walks through what an in-house healthcare SDR really costs, using published benchmarks, and when a managed program is the better buy.

The headline number: compensation

Start with what you'll pay the rep. According to The Bridge Group's 2025 SDR Models, Motions & Metrics report, which surveyed 351 B2B companies, median SDR on-target earnings are $80K, split 68:32 between base ($55K) and variable ($25K). The healthtech posting above sits well above that median, which tells you where competitive healthcare offers are heading.

OTE also isn't what reps actually earn. Bridge Group found that only 60% of SDRs hit quota, the lowest share in the study's history. That can make variable pay look cheaper on paper, but it also means a big chunk of your team is underperforming the plan you budgeted against. Benefits and payroll costs

Salary and commission are only part of what an employee costs. The U.S. Bureau of Labor Statistics' Employer Costs for Employee Compensation release for June 2026 shows that for full-time private industry workers, wages and salaries accounted for 68.5% of employer compensation costs, and benefits accounted for the remaining 31.5%.

As a rough illustration, if an SDR's pay package matches that national average mix, each $1 of cash compensation comes with about another $0.46 in benefits and payroll costs. Your actual number depends on your benefits plan, but it's never zero.

Data and tools

A healthcare SDR can't prospect without a stack. A typical seat includes:

  • LinkedIn Sales Navigator. LinkedIn's published pricing lists Core at $1,079.88 per license per year and Advanced at $1,799.88 per license per year on annual billing.
  • Contact and firmographic data for health systems, payers, and provider groups, which you'll need to keep current as healthcare titles and org charts change.
  • A sales engagement platform and dialer to run multichannel sequences.
  • Intent and account research tools to decide which accounts to work first.
  • CRM seats and administration to keep routing, deduplication, and reporting working.

Each is a separate contract, renewal, and integration that someone has to buy, connect, and keep clean.

Ramp time: paying for months of no pipeline

New SDRs don't produce on day one. Bridge Group puts average ramp time to full productivity at 3.0 months, the lowest since 2010. That's for B2B companies broadly, most of them SaaS.

Healthcare adds its own learning curve: who buys at a health system versus a health plan, how clinical, IT, finance, and procurement stakeholders interact, and how to talk value without touching PHI.

Turnover: the cost you pay again and again

The SDR seat is built to turn over. Bridge Group reports a median annual SDR attrition rate of 40% (with the middle half of companies between 21% and 57%) and an average tenure of 1.9 years. Of that attrition, 16 points came from promotions, which is a good outcome for the rep but still leaves you with an empty seat.

Do the math: if ramp takes about three months and average tenure is about 23 months, roughly one of every eight months a typical rep spends with you is ramp. And the healthtech posting above advertises an AE promotion path in 6-9 months, which would put a much larger share of a rep's time in ramp.

Every departure restarts the clock: recruiting, onboarding, ramp, and the territory knowledge that walks out the door. Gallup estimates that replacing an individual employee can cost one-half to two times the employee's annual salary, and calls that a conservative estimate.

Management overhead

SDRs need coaching, call reviews, list building, messaging updates, and reporting. Bridge Group found an average of 6.4 SDRs per first-line leader and a median SDR manager OTE of $146K. Spread across 6.4 reps, that's roughly $23K of manager compensation per SDR before the manager's own benefits, and it assumes you have enough reps to justify a full-time manager.

Smaller teams often don't, so a VP of Sales or founder manages them part-time and the cost shows up as lost selling time.
